LOST CASE:
- One attested copy of Green Book page 1, 2 and the latest updated payment page.
- Two sets of attested copies of valid Registration Certificate (RC) .
- Two copies of lost IC, if available. If not, then a proof of Date of Birth document is required.
- Original and attested copy of the ‘FIR’ lodged with police bearing lost IC number.
- Two passport-size color photographs on glossy paper. The background of the photograph should be plain white & the dress should be in dark color.
- Two attested copies of marriage certificate are required if the name of the spouse is to be added.
- Two attested copies of “present address”proof (adhaar/running bank passbook with affixed photo/electricity bill/rent agreement etc.) Please refer the link for more details
Applicants are required to submit the printout copy of the filled-in Online IC application form signed in ink to the Bureau of His Holiness the Dalai Lama, New Delhi within a month, with all required supporting documents.

- ཕྱི་སྐྱོད་ལག་དེབ་རྙིང་པའི་ངོ་བཤུས་གཉིས། དེ་མེད་ཚེ་སྐྱེས་ཚེས་ལག་འཁྱེར་གྱི་ངོ་བཤུས་གཉིས། ཡང་ན། འབའ་ཡིག་ (Affidavit) ནང་དོ་བདག་གི་མིང་དང། ཕ་མའི་མིང་། སྐྱེས་ཚེས་སྐྱེས་ཡུལ་བཅས་འཁོད་པའི་ཡིག་ཆ་ངོ་མ་དང་དེའི་ངོ་བཤུས་གཅིག
- ཉེན་རྟོག་ལས་ཁུངས་སུ་ཕྱི་སྐྱོད་ལག་དེབ་བོར་བརླགས་ཤོར་བའི་ཞུ་གཏུགས་བྱས་པས་ (FIR) ཡིག་ཆ་ངོ་མ་དང་ངོ་བཤུས་གཅིག བོར་བརླགས་ཞུ་གཏུག་ཡིག་ཆའི་ནང་ཕྱི་སྐྱོད་ལག་དེབ་ཨང་གྲངས་ངེས་པར་འཁོད་ཡོད་དགོས།
- རྒྱབ་ལྗོངས་དཀར་པོ་དང་སྟོད་གོས་ཁ་མདོག་ནག་ཤས་ཆེ་བའི་(Dark color) རང་ཉིད་ཀྱི་ད་ལྟའི་ཕྱེད་པར་ཚོན་ལྡན་ཅན་གཉིས་ Glossy ཤོག་བུའི་སྟེང་པར་ཤུས་དགོས་རྒྱུ། (4.5×3.5)
- སོ་སོའི་ལག་དེབ་སེར་པོ་སྟེང་བཟའ་ཟླའི་མིང་གསར་སྣོན་ཞུ་དགོས་ཚེ་གཉེན་སྒྲིག་ལག་འཁྱེར་གྱི་ངོ་བཤུས་གཉིས།
- དྲྭ་རྒྱའི་ཐོག་བཀང་པའི་འགེངས་ཤོག་སྟེང་དོ་བདག་གི་ས་རྟགས་འཁོད་པའི་དཔར་བསྐྲུན་ངོ་བཤུས།
- འགེངས་ཤོག་ནང་བཀང་སོ་སོའི་ད་ལྟའི་སྡོད་གནས་ཁ་བྱང་གྱི་ཁུངས་སྐྱེལ་ཡིག་ཆའི་ངོ་བཤུས་གཉིས། (དཔེ་ན། Aadhar, དངུལ་ཁང་ལག་དེབ། ཁང་གཡར་འགན་ཡིག སོགས་ཀྱི་ངོ་བཤུས། གཞན་ངོས་ལེན་ཡོང་པའི་ཡིག་ཆ་ཁག་གི་ཞིབ་ཕྲ་དྲ་ཐོག་གཟིགས་གནང་ཡོང་བ་ཞུ། passportindia.gov.in བུ་ཕྲུག་ལོ་ན་མ་སོན་པ་ཡིན་ན་ཕ་མ་གང་རུང་གི་ཁུངས་སྐྱེལ་ཡིག་ཆ་གནང་དགོས།
