IC Damaged Case:
- One attested copy of Green Book page 1, 2 and the latest updated payment page.
- Two sets of attested copies of valid Registration Certificate (RC).
- Latest original IC with two sets of attested copies of the front and last inside covering page.
- An application from the applicant clearly stating the cause of the damage (Application should be addressed to Regional Passport Officer, RK Puram, New Delhi).
- Court Affidavit in original affirmed before Magistrate bearing IC Number with one attested copy.
- Two passport-size color photographs on glossy paper. The background of the photograph should be plain white & the dress should be in dark color.
- Two attested copies of marriage certificate are required if the name of the spouse is to be added.
- Two attested copies of “present address” proof (Aadhaar/running bank passbook with affixed photo/electricity bill/rent agreement etc.) Please refer the link for more details.
Note: Application may be subject to Inquiry if the photo or the NORI on the IC is damaged beyond recognition.
Applicants are required to submit the printout copy of the filled-in Online IC application form signed in ink to the Bureau of His Holiness the Dalai Lama, New Delhi within a month, with all required supporting documents.

- དང་བླངས་དཔྱ་དངུལ་ལག་དེབ་ཀྱི་ཤོག་གྲངས་དང་པོ་དང་། གཉིས་པ། འབུལ་འབབ་གཙང་ཕུལ་ཟིན་པའི་མཐའ་མའི་ངོ་བཤུས།
- དུས་འགྱངས་ཞུས་པའི་སྐྱབས་བཅོལ་ལག་འཁྱེར་གྱི་ངོ་བཤུས་གཉིས།
- ཕྱི་སྐྱོད་ལག་དེབ་རྙིང་པ་ངོ་མ་དང་དེའི་ངོ་བཤུས་གཉིས།
- ལྡི་ལི་ RPO ལས་ཁུངས་སུ་དོ་བདག་ནས་ས་རྟགས་འཁོད་པའི་ལག་དེབ་སེར་པོ་སྐྱོན་ཤོར་ཕྱིན་པའི་སྙན་ཞུ་འབུལ་དགོས།
- སྐྱོན་ཤོར་ཕྱིན་པའི་འབའ་ཡིག་ (Damage Affidavit) ངོ་མ་དང་ངོ་བཤུས་གཅིག འབའ་ཡིག་ནང་ལག་དེབ་སྐྱོན་ཇི་ལྟར་ཤོར་ཚུལ་ཞིབ་ཕྲ་འཁོད་དགོས།
- རྒྱབ་ལྗོངས་དཀར་པོ་དང་སྟོད་གོས་ཁ་མདོག་ནག་ཤས་ཆེ་བའི་(Dark color) རང་ཉིད་ཀྱི་ད་ལྟའི་ཕྱེད་པར་ཚོན་ལྡན་ཅན་གཉིས་ Glossy ཤོག་བུའི་སྟེང་པར་ཤུས་དགོས་རྒྱུ། (4.5×3.5)
- དྲྭ་རྒྱའི་ཐོག་བཀང་པའི་འགེངས་ཤོག་སྟེང་དོ་བདག་གི་ས་རྟགས་འཁོད་པའི་དཔར་བསྐྲུན་ངོ་བཤུས།
- འགེངས་ཤོག་ནང་བཀང་སོ་སོའི་ད་ལྟའི་སྡོད་གནས་ཁ་བྱང་གྱི་ཁུངས་སྐྱེལ་ཡིག་ཆའི་ངོ་བཤུས་གཉིས། (དཔེ་ན། Aadhar, དངུལ་ཁང་ལག་དེབ། ཁང་གཡར་འགན་ཡིག སོགས་ཀྱི་ངོ་བཤུས། གཞན་ངོས་ལེན་ཡོང་པའི་ཡིག་ཆ་ཁག་གི་ཞིབ་ཕྲ་དྲ་ཐོག་གཟིགས་གནང་ཡོང་བ་ཞུ། passportindia.gov.in བུ་ཕྲུག་ལོ་ན་མ་སོན་པ་ཡིན་ན་ཕ་མ་གང་རུང་གི་ཁུངས་སྐྱེལ་ཡིག་ཆ་གནང་དགོས།
