CHANGE OF NAME IN IC:
- One attested copy of Green Book page 1, 2 and the latest updated payment page.
- Two sets of attested copies of valid Registration Certificate (RC) with the corrected name.
- Latest original IC with two sets of attested copies of the front and last inside covering page.
- If the latest IC is issued by a Foreign Mission, the applicant must also submit the original IC and two sets of attested copies of the last IC issued by the Regional Passport Office (RPO), New Delhi.
- Two passport-size color photographs on glossy paper. The background of the photograph should be plain white & the dress should be in dark color.
- Original Advertisement in two newspapers (one national and one local) with an attested copy each.
- Two attested copies of “present address”proof (adhaar/running bank passbook affixed photo/electricity bill/rent agreement etc.) Please refer the link for more details
Applicants are required to submit the printout copy of the filled-in Online IC application form signed in ink to the Bureau of His Holiness the Dalai Lama, New Delhi within a month, with all required supporting documents.

- རྒྱབ་ལྗོངས་དཀར་པོ་དང་སྟོད་གོས་ཁ་མདོག་ནག་ཤས་ཆེ་བའི་(Dark color) རང་ཉིད་ཀྱི་ད་ལྟའི་ཕྱེད་པར་ཚོན་ལྡན་ཅན་གཉིས་ Glossy ཤོག་བུའི་སྟེང་པར་ཤུས་དགོས་རྒྱུ། (4.5×3.5)
- རྒྱ་གར་རྒྱལ་ཡོངས་ཚགས་པར་གང་རུང་གཉིས་ཀྱི་ཐོག་དོ་བདག་གི་མིང་ནོར་བཅོས་ཁྱབ་བསྒྲགས་བྱས་པའི་ཚགས་ཤོག་རེ་རེ་དང་དེའི་ངོ་བཤུས་རེ་འབུལ་དགོས།
- དྲྭ་རྒྱའི་ཐོག་བཀང་པའི་འགེངས་ཤོག་སྟེང་དོ་བདག་གི་ས་རྟགས་འཁོད་པའི་དཔར་བསྐྲུན་ངོ་བཤུས།
